Flying out of Georgia with a pet: where did you get the export certificate?

Has anyone done this recently from Tbilisi? Please include the destination, airline, month and year, where you got the export document, how early you booked and what was actually checked at the airport. Rules change, so old answers can be misleading. Everyone should still confirm the current requirements with the destination country, airline and Georgian authorities. Starter thread from a HAV HAV community contributor.

It would be useful to know whether the appointment and document were handled in English, Georgian or Russian too.

Please mention the crate size and airline as well. The paperwork may be correct while the carrier still refuses the transport box.
